SUMMARY AND OBJECTIVE:The State Registered Nursing Assistant (SRNA) delivers direct care to the resident within the scope of practice for a SRNA and according to the daily care plan for each resident. The SRNA assists the resident with activities of daily living (ADLs) and is a vital caregiver in the delivery of care and services to the resident. The SRNA works under the direction and supervision of licensed nurses to assure that quality care and services are delivered according to a comprehensive care plan and the physician orders for each resident. Resident rights and resident choice are honored at all times by the SRNA in the daily care of the resident. The SRNA follows all policies of the facility, adheres to state and federal regulations, and maintains resident safety. The SRNA is a critical member of the team in assuring that a resident attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.REQUIRED EDUCATION AND EXPERIENCE:Certified by the State as a Nursing Assistant or eligible for certification.Experience in the provision of comfort and care.COMPETENCIES AND QUALIFICATIONS:Demonstrate aptitude for empathetic care and working with the elderly.Knowledgeable of Resident Rights for Long Term Care Facilities.Knowledge and skill in delivering direct care to a resident.Possess patience and good listening skills.Exhibit a positive attitude.Ability to participate in continuing education.Ability to communicate with other staff members and other disciplinary team members to coordinate care of the resident.Communicate issues and concerns and provide updates to DON.Ability to obtain certifications related to responsibilities.Ability to complete daily assignments under the direction of a licensed nurse.Organized and self-directed.Ability to follow policies, procedures, systems and programs.Competent oral and written communication skills.Aptitude for a culture of accountability and attention to detail.Competent in electronic patient documentation of care.Ability to relate positively, effectively and appropriately with others.Ability to work and communicate within a teamwork environment.Comfortable with working closely with individuals on a one-to-one basis.Ability to be fiscally responsible with facility resources.Meet all health requirements imposed by law.Ability to obtain CPR certification.Ability to perform under stressful situations.ESSENTIAL JOB FUNCTIONS:Able to walk and stand for extended periods of time.Able to exert up to 50 pounds of force frequently.Able to perform frequent bending, twisting, stooping, pushing, pulling, standing and walking while giving hands-on assistance with resident care.Able to speak and hear clearly.Ability to cope with mental and emotional stresses.Able to endure exposure and effects of odors, dust, disinfectants, and other air contaminates.Able to effectively communicate with hostile and emotionally upset residents, family members, personnel, visitors, etc.Able to work in a bright and noisy environment with intermittent distractions.Adhere to employment policies.Honor Resident Rights and choice as related to Certified Nursing Facilities.General knowledge of current state and federal regulations.Maintains a working knowledge of policies, procedures, care systems and practices of the Center.Able to operate a keyboard and enter data into computer software programs.Able to perform hands-on assistance with resident care within education and experience qualifications.Able to handle equipment, supplies and materials related to job responsibilities.JOB RESPONSIBILITIES:Comply with the employee's Code of Ethics.Comply with the facility Compliance and Ethics program.Provide direct care and service to each resident as scheduled while honoring the rights of the resident to assure that the resident receives care and services according to facility policies and regulatory standards.Assist in the resident admission process.Listen and communicate effectively to each resident.Perform resident bathing, toileting, oral hygiene, grooming, perineal care, making beds, vital signs, weighing, and monitoring devices in accordance with the resident care plan and facility policies and procedures.Provide assistance for transporting, escorting, ambulating, transferring, turning, repositioning, and lifting in accordance with the resident care plan and facility policies and procedures.Perform range of motion exercises (active and passive).Accurately record any required documentation to include care and treatment provided and response or lack of response to care.Demonstrate sensitivity to the physical and mental limitations of each resident.Answer and respond to call lights.Assure that supplies and equipment are obtained prior to resident care.Adhere to the facility infection control policies and practices.Maintain the resident room in a clean, sanitary and orderly manner.Identify resident's personal clothing, personal possessions and takes inventory of personal possessions.Handle, store and transport clothing and linens according to facility policies.Request housekeeping and laundry services as needed.Support clinical services in assuring resident care is delivered according to a care plan and physician directed care.Attend in-service education as requested by the facility and all other education as required by law. Assure that all work areas are maintained in a clean, orderly, and professional manner and according to facility and regulatory requirements.Assist the resident request for assistance by conducting frequent rounds and responding to resident calls.Assist in investigations as requested.Accompany resident in transportation as requested.Participate in interdisciplinary care plan meetings as requested.Provide clinical expertise in the management of medical and psychiatric emergencies.Report changes in a resident to the supervising nurse immediately.Assures food is transported in a sanitary manner and is appropriately covered.Completes set up of resident food trays, serves food at appropriate temperature, and verifies accuracy of food delivered to resident.Obtain meal substitutes for residents who request them.Provides assistance with eating.Returns trays to dietary department after meals in an effective and timely manner.Report residents to the supervising nurse who are refusing meals.Provide adequate assistance in personal and oral hygiene.Assist in meal delivery to dining rooms, resident rooms and other dining areas.Monitor and record the intake of resident meals and snacks.Monitor and record the intake of fluids and report to the supervising nurse.Complete the duties of the nursing assistant assignment.Complete care according to the nursing assistant care plan.Participate in the facility Disaster Plan and Emergency Preparedness Plan, as requested.Work with other facility disciplines for the integration and coordination of resident care and services.Communicate all issues, concerns, and updates to the nursing supervisor and others as may be required by facility policy.Knowledgeable of federal, state and local regulations.Participate in the correction of survey deficiencies.Complete documentation according to facility requirements, job functions and responsibilities.Adhere to all policies and procedures as established by the facility.Participate in the implementation of written policies and procedures.Assist in the performance of meeting state and national standards.Participate in providing and promoting resident safety, quality of care, resident rights, resident choice, and respect of each resident.Assure that confidentiality of medical records and resident care are maintained.Knowledgeable of the Quality Assessment and Performance Improvement (QAPI) program and participate when requested.Work with other departments on activities that contribute to the enhancement of care and services.Knowledgeable of the facility infection prevention and control policies.Report safety concerns or abuse concerns to the Administrator and Social Worker immediately.Maintain current knowledge of SRNA standards and practice.Report any patient care, documentation or survey concerns to the Director of Nursing immediately.Maintain compliance with the Abuse Prohibition program including the reporting of any allegation of resident abuse.Adhere to employment policies and report breaches of employment policies.Assist new employees in following established facility policies and procedures and assist in training of employees as requested.Follow procedures in the care of equipment and in compliance with facility policies.Follow MSDS (Material Safety Data Sheet) data sheets.Adhere to OSHA (Occupational Safety and Health Administration) rules.Follow waste disposal procedures in accordance with center procedures.Obtain continued education and certifications to assist in the essential functions and responsibilities of the job.SUPERVISORY RESPONSIBILITY:No supervisory responsibility required of the position.WORK ENVIRONMENT RISKS:Subject to hostile and emotionally upset residents, family members, personnel, visitors, etc.Subject to hot and cold temperatures.Subject to loud noises and bright environment.May work beyond normal working hours and on weekends and holidays.May be exposed to the effects of odors, dust, disinfectants, and other air contaminates.May be exposed to infectious waste, diseases, conditions, etc., including TB, AIDS, and Hepatitis.May be subject to the handling of and exposure to hazardous chemicals.Works in patient care areas and areas related to care.May be exposed to disagreeable situations due to human interactions.PHYSICAL DEMANDS:Handling resident care equipment, supplies and materials.Standing and walking for extended periods of time.Work for limited amounts of time on computer.Ability to exert up to 50 pounds of force occasionally.Extensive Emergency Preparedness Plan and these may require transportation of residents.Assist and participate in emergency drills.CONFIDENTIALITY AND HIPPA COMPLIANCE:The facility shares Protected Health Information (PHI) with the employee enabling the employee to perform job functions while providing services to the residents in the facility. The employee is responsible for ensuring that confidentiality is protected for all PHI and under the rules set forth by the Health Insurance Portability and Accountability Act (HIPAA).INDHS
