Job Description:
Company Overview Commonwealth Community Based Services is
currently hiring Aides/Caregivers in Bourbon County, KY (including
Paris, Winchester & surrounding areas) to provide quality,
compassionate care for patients in their homes. We offer a Loyalty

you'll be able to earn PTO hours if you work 32 hrs/week or more!
The Caregiver / Aide is responsible for providing patients with
in-home personal care and designated health related services to
maintain the patient's physical and emotional well being, while
following the written plan of care, Medicare/Medicaid regulations
and agency policies and procedures.Commonwealth Community Based

Essential FunctionsProvides or assists clients with activities of
daily living including: bathing in bed, tub or shower; care of hair
including shampoo, combing and brushing; care of teeth and mouth
including denture care; nail care, filing only; skin care including
pericare and applying lotion; transfer of patient from bed to chair
and to wheelchair. Transfer of patients on and off bedpan, commode
and toilet. Assist client in preparing for bed. Position patient in
bed as directed. Assists with ambulation including with cane,
walker and crutches. Personal care dressing of client. Medication
reminder.Live-in and Sleep Over's: Secures clients home for the
night and provides reassurance by being present at the client's
home during the night time hours. (Washington and Oregon does not
provide this service)Utilize the E.V.V. (Electronic Visit
Verification) program as applicable.Assists with household tasks
directly essential to client's personal care.Accompanies client to
medical appointments or shopping if necessary and performing other
essential errands.Measures and records oral, rectal, and auxiliary
temperatures, pulse and respiration, and blood pressure when
ordered within the training limitations of the aide.Prepares meals
as required based upon client's preferred diet. May need to feed
client if required.Provides clean and safe environment within the
home including safe use of equipment such as side rails, oxygen,
wheelchairs, Hoyer lifts, etc.Ensures confidentiality of client
records and information.Records and reports activities related to
personal care responsibilities, as required.Responds promptly to
client requests.Documents all client related activities in a timely
manner.Reports observations of the client's condition to the Branch
Manager or Office Assistant. Notifies the Branch Manager
immediately if any incidents or accidents occur.Participates in
performance improvement in-services.Drives safely, complies with
traffic regulations, and wears a seat belt when business travel is
required.Reacts to change productively and performs other
job-related tasks and duties as assigned. Education &
ExperienceEducation DesiredSix months experience in home care
preferred.License RequirementsAccess to public transportation OR
dependable vehicle with a current driver's license and vehicle
insurance.State Specific RequirementsAR: Must have completed a 40
hour training course approved by the Arkansas Department of Health
and be a certified personal care aide.FL: Successful completion of
at least forty hours of training in areas required by Agency for
Health Care Administration OR successfully passed the home health
aide competency test (Form AHCA 3110-1007 created by AHCA).KY:
Current CPR Certification.LA: Current CPR and First Aide
Certification.MD: Must be certified by the Maryland Board of
Nursing as a certified nursing assistant. Current CPR
Certification.MO: Must complete 20 hours of basic training within
30 days of employment. Eight hours must be provided by the agency.
An additional 12 hours may be waived if similar training within the
last 12 months or if employed as an aide in a home health agency at
least half-time for six months or more within the last 12months. If
certified nursing assistant, basic training requirement exempt,
with the exception of a 2- hour orientation.NY: Must have completed
a training program in home health aide services OR equivalent exam
OR one full year experience in providing personal care services
through a home care agency OR training program in personal care
services.OH: Must have successfully completed an approved Ohio
Department of Health State Tested Nurse Aide (STNA) training
program and competency, OR meet the qualifications to be Medicare
Certified Home Health Aide, OR have at least one (1) year
supervised employment experience as a home health aide or nurse
aideand has successfully completed a competency evaluation, OR has
successfully completed a Council on Aging Learning Advantages
(COALA) Home Health training program, a certified vocational or
other sixty (60) hour training and competency evaluation program
that meets the requirements listed in rule
173-39-02.11(C)(3)(a)(v)(b). Current CPR and First Aid
CertificationPA: Must successfully complete a home health training
program OR nurse aide certification program sponsored by the PA
Department of Education.TN: Must have evidence of medication
assistance training if performing this function; CPR and First Aid
Training required for services provided under Choices/Employment
and Community First Choices Program.WI: Must have 40 hours training
or 6 months of equivalent experienceCT, NC, TX, VA: No other state
specific requirementsSkill RequirementsAbility to work flexible
hours as required to meet identified patients' needs.Ability to
function in any home situation regardless of age, race, creed,
color, sex, disability, or financial condition of the patient.Able
to work independently.Ability to multi-task, self-directed, good
time management skills, courteous, and common sense.Good
communication, writing, and organizational skills.Must possess the
disposition and personality to work harmoniously with a wide
variety of people and their families keeping personal matters
