Job Description:
Job Description Central Health Management, Inc. is seeking a
qualified Nurse Practitioner to provide primary care coverage in
long-term care facilities in the Lexington, KY area and provide
follow up for patients undergoing in-home therapy. This role is
expected to work 16 to 18 days per month and will expand to full
time. In this role, you will round on patients in skilled nursing
centers in the Lexington area, ensuring the patient is evaluated
and being treated in accordance with the established plan of care.
Adjusting the care plan to address new or changing issues and when
appropriate adjusting medications. This position interacts with the
building staff to ensure a high level of quality care and patient
safety are top priorities. Additionally, as needed this patient
will perform a follow up visit to in-home patients within 48 hours
of their receiving in-home therapy. This is a 1099 contracted role
and is expected to earn $120,000 or more annually for 12 to 18 days
of work per month. To qualify, you will have: - MSN or DNP from an
accredited Nurse Practitioner curriculum. - Active and unencumbered
license to practice as a Nurse Practitioner in Kentucky. - Active
and unencumbered DEA license to prescribe in Kentucky. - BLS and/or
ACLS certification. - Excellent communication and collaboration
skills. - Strong attention to details, and ability to resolve
problems in a timely manner.
