Facility:-- Saint Joseph Hospital, Lexington, KY

Job Summary

RN Clinical Coordinator Role is responsible for actively coordinating and
monitoring all clinical aspects of the traditional cardiac rehab
intervention.-- The RN ensures the
appropriate selection of participants and monitors participant safety with
active oversight of the Medical Director.--
The RN assesses participant's clinical response to the interdisciplinary
treatment plan and funnels communication to the appropriate specialist and / or
Medical Director or referring physician.--
Provides leadership of case conferences and assures safe care transition
of participants upon graduation.-- The RN
functions as the super user of the electronic health record, Diaweb, and
Versacare and participates as a member of the transdisciplinary team to ensure
the quality and clinical effectiveness of the program.


Essential Functions:

health assessment at baseline on all participants. Performs an assessment on
every participant before each intervention session to ensure safe participation.

    Knowledge or Abilities critical to this role:


    • Current
      knowledge of the latest science and best practices around wellness, prevention,
      screening, and behavior modification for successful lifestyle changes and
      optimal health outcomes for participants.
    • Ability
      to operate and teach operation of telemetry monitors, defibrillator, aerobic
      exercise equipment, weight machines, free weights, audiovisual equipment,
      glucose meters, pulse monitors/oximeters.--
      Able to operate computer operating systems such as Cerner, VersaCare,
      McKessen, STAR.




      • Must
        be proficient in both written and verbal communication and have excellent
        English-language speaking skills.
      • Excellent
        communication skills and ability to connect with participants, referral sources
        and the general community.
      • Exhibits
        excellent interpersonal communication skills with compassion, empathy, and
        sensitivity to participants needs.
      • Excellent
        public speaking skills and knowledge of adult education and coaching



      • Requires strong skills in reasoning, analysis, and


        Computer Skills:

      • Requires
        skill in Microsoft Office products as well as knowledge of scheduling and
        billing software.

        Job Requirements / Qualifications


        Education /
        Accreditation / Licensure (required & preferred):

        Education:-- BSN preferred, ADN required.--

        Licensure:-- Registered Nurse in the state of Kentucky


        (required and preferred):

        Current certification in BLS and ACLS required

        Formal education/training in cardiac
        rehabilitation and / or exercise physiology preferred

        Minimum of two years clinical experience in
        adult cardiovascular nursing and/ or cardiac rehabilitation
