Job Summary / Purpose
Works collaboratively with physicians, staff and other
health care professionals within his/her Division to coordinate the
service of selected patient populations across the continuum within
care setting. The role works collaboratively with patient, family,
and other members of the health care team to achieve the highest
clinical outcomes with the most cost effective use of available
Social Worker supports responsibility for an interdisciplinary
process to meet
the psychosocial and transitional needs of the patient and family.
Worker is knowledgeable about various age-related care protocols
within their scope of practice. The Social Worker is an integral
member of the
health care team as well as the Divisional Care Management team.--
In addition, the position collaborates with
the Divisional Care Management team on system-wide quality
improvement/performance improvement initiatives.
Essential Key Job Responsibilities
- Conducts a psychosocial assessment on
identified priority patients in collaboration with family.
- Outline available strategies and services to
assist in addressing psychosocial care needs, and to make

screen, Health Literacy screen, Mental Status screen, and Social
screen as well as other specialized screens and evaluative

needed programs and services.
- Assesses need for services through
multidisciplinary rounds on patients and collaboration with
other interdisciplinary team members.

Required Education for Staff Job Levels
Master or Doctorate of Social Work required
from accredited college.
Required Licensure and

Required Minimum Knowledge, Skills and Abilities
Demonstrated experience in case
management, discharge planning, and transfer coordination.--
Must have excellent computer skills and
ability to learn new systems.
Knowledgeable regarding issues of
chronic illnesses, loss/grief issues, change process, family
relationship principals, strength based interventions and community
Possesses knowledge of public and
private welfare and health agencies available to serve patients
Must have strong organizational (time
management) skills, strong interpersonal skills, the ability to
multiple priorities with strong attention to detail
Knowledge of and practical use of good
business English, spelling, arithmetic, practices and the ability
communicate effectively using written and verbal skills. Proficient
communications and internet usage along with basic use of Microsoft

Ability to work autonomously within

Minimum 3 years clinical experience as a
social worker required.
Knowledge of Cerner electronic health
record preferred, for locations with Cerner.--
Case management certification (CCM or ACM) preferred.-- Knowledge

Care Guidelines) authorization criteria preferred.
